You’re juggling phone calls from doctors.
Chasing down medication changes.
Trying to remember what happened during the last hospital stay…
And somewhere in the middle of all that, you're just trying to hold it together.
Then you walk into an appointment feeling like you’re still missing pieces of the puzzle - and when someone asks:
“What medications are they on?”
Your mind goes blank.
That moment of panic?
That’s what this planner was built to prevent.
The Medical Assistance Planner is a printable, easy-to-use system designed to organize everything you need to manage your loved one’s care - with less stress and more clarity.
Created by Laura Wilkerson, Nurse Practitioner and caregiver advocate, this planner was born out of both professional insight and personal experience.
It’s the tool she wished her own family had - and now, it’s available for yours.
✅ Advocate for your loved one with confidence
✅ Stay organized for emergencies, transitions, and long-term care
✅ Reduce mental load and feel prepared at every step
It’s easy to think, "I’ll just figure it out when something happens."
But when you're juggling doctor calls, medication changes, and hospital discharges...
Trying to gather the right information in a crisis can feel impossible.
I’ve lived it - not just as a nurse practitioner, but as a daughter.
Here’s why I created the Medical Assistance Planner:
More than 20 years ago, my dad had a routine hernia repair using a surgical mesh that was supposed to make life easier.
Instead, it became the start of a long, complicated medical journey - one that has shaped how I see caregiving forever.
🛠️ Over the years, my dad endured multiple surgeries to correct mesh complications.
❤️ He’s had two heart attacks.
🫀 The most recent one led to open-heart surgery and life support.
After his surgery, my dad needed close coordination between the hospital, rehab center, and home care teams.
But instead of seamless care, there were critical failures at every turn:
❌ His hospital discharge included incorrect medications.
❌ Prescriptions were never sent to the pharmacy.
❌ Rehab failed to provide basic contact information for his wound care team.
❌ It took 11 days after discharge for a home health nurse to even reach out - because they had the wrong phone number.
And it wasn’t a one-time mistake.
Years earlier, after another surgery, his hospital was hit by a cyberattack.
His medical records were lost forever.
If my dad had been using the Medical Assistance Planner all along, things could have looked very different.
He would’ve had a clear, organized record of:
📋 Every surgery, complication, and specialist involved.
📋 All medication changes — and the reasons behind them.
📋 Questions to ask at every follow-up appointment.
📋A simple list of phone numbers, referrals, and timelines — ready at a moment’s notice.
Just this week, my mom was concerned about a chest X-ray before my dad’s procedure.
She didn’t have prior imaging on hand.
And even as a nurse practitioner - even as his daughter! - I couldn’t give her a clear answer.
Imagine how different that conversation could have been if everything had been documented and ready.
As a healthcare provider, I’ve seen the system fail families far too many times.
That’s why I created the Medical Assistance Planner — to give caregivers a system.
Because being prepared isn’t just about being organized
✅ It's about being heard,
✅ Feeling empowered,
✅ And protecting the people you love.
After helping hundreds of families - and living through my own - I designed the MAP to cover the most important information every caregiver needs at their fingertips.

✅The MAP gives you a clear, easy-to-follow system, built from real-world experience.

I created this planner not just as a medical professional, but as someone who’s lived this on both sides.
I’ve had to care for my own parents. I’ve watched the overwhelm, the scrambling, the grief that comes with
not being prepared.
That’s why the Medical Assistance Planner isn’t just a “nice idea.”
It’s a tool I know works - because I’ve used it with thousands of families, and in my own.
But I’m not just a nurse practitioner. I’m also a daughter who’s sat in the ER wondering what the last test result said.
I’ve held the hands of families during hospice, heart failure, sudden strokes - and almost every time, I’m the one trying to track down records, meds, and timelines.
But what I’ve learned is: if families had this
one thing, there would be no delay. No second-guessing.
This planner is where advocacy begins.
It was made for them, for me - and now,
it’s for you.
Listen - I know the stress. I’ve seen the tears. I’ve felt the weight of carrying it all in your head.
That’s why I created the Medical Assistance Planner, to give you a starting place.
So you never have to start from scratch again.
So you don’t have to
remember everything
Next time you go to a doctor’s appointment, you open your binder, flip to your "Doctor Visits" page, and confidently share your loved one’s health history.
No more guessing.
No more panic.
Just calm, collected caregiving.
We know not everyone is super comfortable with online tools - and that’s okay! Here are answers to some common questions to make everything feel easy and stress-free.
🖥️ What exactly am I buying?
You’re purchasing a digital planner - a downloadable PDF you can save to your computer or device and print at home. It’s not a physical book that will be shipped. As a bonus, you'll also receive a PDF fillable copy of the planner - so you can type on the pages before printing (if you want).
📥 How do I get the planner after I buy it?
Right after your purchase, you’ll get an email with a link to download the file right away. Just click the link and save it to your computer or phone.
🖨️ Do I need special software to use it?
Nope! It’s a regular PDF file. If you’re on a computer or tablet, it’ll likely open automatically. If not, you can use free programs like Adobe Reader (which is probably already on your computer).
If you want to type in the fields, download the "fillable PDF" version of the planner. This is included at no extra cost in your order.
🧾 Can I print it?
Yes! The planner is designed to be printed and placed in a binder. You can print it all at once or just the pages you need. Many people keep it in a 3-ring binder with tabbed dividers.
🧑⚕️ What if I want to share this with someone helping me?
Yes, absolutely - you can share your completed planner with a family member, caregiver, or healthcare provider. That’s what it’s designed for!
